A-133 Audits and Yellow Book Services
All government and not-for-profit entities that expend more than $500,000 a year in federal funds are required by the Office of Management and Budget (OMB) to submit to an A-133 audit. If you are spending less than $500,000 per year in federal funds, you still may need an audit conducted according to yellow book standards.
Unified Chart of Accounts

The Unified Chart of Accounts for nonprofit is a chart of accounts designed so that a nonprofit organization can can quickly and reliably translate their financial statements into the categories required by the IRS on Form 990. It promotes consistency and financial reporting standards based on the criteria set forth in the Yellow Book auditing standards.

I recently decided to check the status of several nonprofits/charitable organizations in my area 
I began by checking the organization's status using the IRS online pub 78. In most cases, the IRS status was up to date, bear in mind that a nonprofit will not generally lose its exempt status unless it fails to file annually for three consecutive years.  I then went to the Massachusetts nonprofit document search website; this not only provides a listing but also the scanned images of the documents as well. I was shocked to discover many of the nonprofits I checked where missing prior year forms or had not filed at all with the "State of Massachusetts." Please remember the site is intended as a informational tool only. There could be several reasons why the documents are not shown on the site.
